In high season that is not a good idea. The ticket office sells from the same quota as the official website. Once the slots are gone online, there are none left at the desk either.

No. Santa Maria del Fiore cathedral is free. The Brunelleschi Pass only covers the paid monuments: dome, bell tower, baptistery, museum and Santa Reparata.
The dome has 463 steps, no lift and some very narrow passages. The bell tower has 414 steps with wider stairs and landings. If you only want one climb, the Giotto Pass without the dome is enough.
